Weight Programme Privacy Notice

EMPOWER Weight Management programme combines medication with educational resources and clinical support to help people lose weight and keep it off in the long-term.

EMPOWER is part of Spirit Health who employs a specialist team of healthcare professionals, service delivery staff and experienced administrators to:

  • Manage the onboarding process
  • Create tailored, educational content
  • Support patients with any clinical queries associated with their weight loss
  • Collect and review key clinical metrics pre and post-course

The EMPOWER programmes are run and managed by Spirit Health.

What information is being collected for the weight management programme by Spirit?

  • General contact information: Name, Email, Physical Address, Telephone number, Gender, Date of birth,
  • Clinical information: NHS number, GP Practice, basic medical history including height, weight, BMI, medication and smoking
  • Ethnicity
  • General health questions e.g. mobility, pain levels, dementia and self-care
  • Blood test results
  • Equality and diversity monitoring (this is optional)

How is it collected?

  • Directly from the patient’s healthcare professional e.g. GP or Practice Nurse
  • Directly from the patient

Why is it being collected?

This information is being collected in order to decide if the patient is eligible for the programme and to deliver the weight management service effectively and safely. It allows us to tailor the programme to individual needs and support clients in achieving their health goals. Personal information may also be used, with consent, to:

  • manage attendance and participation in the programme
  • contact a client’s GP, where appropriate, to share relevant progress such as goal setting or attendance and medication prescribed
  • conduct follow-up reviews to support long-term outcomes

We only collect the information necessary for these purposes and rely on your consent and/or the need to fulfil our service agreement with you as the lawful basis for processing.

The information is collected for us to deliver the weight management programme and review the patient throughout the programme to assess the impact upon the patient.

How will it be used?

  • To invite the patient onto the programme
  • To deliver the programme
  • Follow progress of the individual for however long they are on the programme
  • To liaise with the GP to obtain medical records

Who will it be shared with?

  • The patient
  • Programme facilitator and administrators
  • The programme’s healthcare team

Out of hours

Patients may also call and leave a message on our answering machine. This information is then emailed to us. The information would be the patient’s query it would be related to our weight management service.

Where will the data be stored?

All information collected is stored electronically in a secure accredited network. Paper documents are kept in a locked cupboard and destroyed one year after the patient leaves the service.

Automated decisions

No automated decision making is used with the data collected.

Your rights as a data subject

As a data subject, you have a number of rights. You can:

  • Access and obtain a copy of your data on request;
  • Require the organisation to change incorrect or incomplete data;
  • Require the organisation to delete or stop processing your data, for example where the data is no longer necessary for the purposes of processing; and
  • Object to the processing of your data where Spirit Health is relying on its legitimate interests as the legal ground for processing.

The Data Protection Officer (DPO)

Spirit have appointed a DPO. This is our Quality and Operations Director – Andy Coxon. The role of the DPO is to assist data ‘Controllers’ and ‘Processors’ to comply with data protection law and avoid the risks that organisations face when processing personal data.
